Help Us Help Others

Diversity Health Center, in collaboration with Hope and Help, Inc. (www.HopeandHelp.org), is pleased to announce our participation in the 340B pharmacy program. Through this plan, funds are made available to us to provide health care to patients who are under- or uninsured. Via this relationship with Hope and Help, we are also able to provide medical case management, assistance with obtaining insurance, help with transportation and food, as well as medical equipment (such as wheelchairs, crutches, etc.).


Here’s how it works. When insured patients get their prescriptions filled at one of our participating pharmacy partners, a portion of the payment for the medication is donated to our “Patient Assistance Fund”. Through this fund, we can offer discounted care to those who cannot afford it. There is no cost to our insured patients. The pharmacy handles all the details.


We are hoping for your help in making this possible. The only thing we ask is that you consider getting your prescriptions filled at one of our local or mail order pharmacy partners. The list of pharmacies is included below, as of 10/1/2018. We are constantly adding new partners.


If you want to be assist us with this, please message us through the portal or give us a call. We will add you to our growing list of patients who are contributing to our “Help Us Help Others” program. We can easily transfer your prescriptions to your new choice of pharmacies.
